Qualifications
- Nursing diploma/degree with registration in SNB.
- Current BCLS/CPR-AED certification and 5+ years of nursing experience.
- Hospice or hospital experience preferred; oncology/geriatrics knowledge is a plus.
Responsibilities
- Provide home care services to patients transitioning from hospital to home.
- Assist with discharge planning and post-discharge care coordination.
- Perform nursing assessments and develop individual care plans.
- Deliver nursing services like feeding, catheter care, dressing changes.
- Document care and coordinate with multidisciplinary team.
- Support administrative tasks as directed by senior staff.
- Rotate shifts, including weekends and public holidays.
- Participate in MDT meetings to ensure seamless patient care.
