Job summary

Fullerton Healthcare Group Pte Ltd is seeking a compassionate Staff Nurse to join our Home Care Services team. You will deliver nursing care in clients' homes, support care planning and coordination, and collaborate with caregivers and healthcare professionals to ensure quality care.

Requirements include a valid SNB practising certificate and 2–3 years of nursing experience. Weekend duties may be required to meet patient needs in home care settings.

Qualifications

• Registered Nurse with SNB practising certificate.
• 2–3 years of nursing experience.
• Experience in community/home care is advantageous.
• Able to work independently and manage home visits.
• Strong communication and interpersonal skills.

Responsibilities

• Provide nursing care to clients in home and community settings.
• Perform wound dressing, medication management, catheter care, stoma care, and tube feeding as required.
• Monitor and assess clients' health conditions and report any changes promptly.
• Administer medications and treatments in accordance with nursing standards and care plans.
• Educate clients and caregivers on care needs and home management.
• Conduct nursing assessments and support the development of care plans.
• Coordinate care with doctors, therapists, caregivers, and other healthcare professionals.
• Support discharge-to-home and long-term care arrangements where required.
• Participate in case discussions and care reviews.
• Maintain accurate and timely clinical documentation.
• Ensure nursing care is delivered according to clinical guidelines and company policies.
• Adhere to infection control, patient safety, and regulatory requirements.
• Report incidents, concerns, and clinical risks appropriately.
• Provide guidance and support to clients and family members.
• Promote health education and self-care where appropriate.
• Support caregivers in carrying out care plans safely and effectively.
• Work closely with Healthcare Assistants, Caregivers, and Operations teams to support service delivery.
• Participate in team meetings, training, and continuous learning activities.
• Provide basic guidance to junior staff and caregivers when required.
