Job summary

Abercare.sg in Singapore is seeking a dedicated Home-Based Care Coordinator to assess clients, develop personalised care plans, and oversee delivery of services.

You will liaise with caregivers and multidisciplinary teams, maintain records, and contribute to quality improvement while supporting enquiries and referrals. Working hours are Monday to Friday, 9am–6pm, with a strong focus on compassionate care and compliance.

Qualifications

• Diploma in Nursing or Social Work (or equivalent).
• At least 2 years in community or geriatric care.
• Strong interpersonal and problem-solving skills.

Responsibilities

• Conduct client assessments and develop personalised care plans.
• Coordinate and monitor home-based care services.
• Supervise and support Community Caregivers to maintain high standards.

• Conduct comprehensive client assessments and develop personalised care plans based on individual needs.
• Coordinate and monitor home-based care services to ensure timely, effective and quality care delivery.
• Work closely with Community Caregivers by providing guidance, supervision and support to ensure high standards of service.
• Liaise with multidisciplinary teams, healthcare institutions and community partners to coordinate holistic care and appropriate follow-up services.
• Maintain accurate client records, care documentation and service reports in compliance with organisational and regulatory requirements.
• Support service quality improvement initiatives, audits and operational projects to enhance care outcomes.
• Manage client enquiries, referrals and operational matters related to Home-Based Care services.
• Participate in community outreach programmes, organisational events and other duties as assigned.

Job Requirement :

• Min Diploma in Nursing / Social Work or other relevant disciplines.
• Min 2 years experience working in community care or geriatric care settings
• Good interpersonal and problem-solving skills
• Proficient in Microsoft Office applications
• Working hours: Mon - Fri 9am to 6pm
