ABER CARE PTE. LTD. in Singapore is seeking a dedicated professional to develop and implement personalised care plans for residents.

You will document progress and coordinate activities to meet care needs, working closely with a multidisciplinary team. The role requires a Diploma/Degree in Social Work, Psychology or Counselling and at least 1 year of healthcare experience, with strong interpersonal and English language skills.

Qualifications

• Diploma/degree in Social Work, Psychology or Counselling.
• At least 1 year of relevant healthcare industry experience.
• Good interpersonal and problem-solving skills.
• Proficient in English and able to converse in dialects or other languages.

Responsabilités

• Create personalised care plans tailored to the needs of the resident.
• Implement care plans and regularly record residents' progress.
• Design and organise programs and activities to address residents' care requirements.
• Prepare and submit review reports and monthly interview records every 6 months.
• Collaborate with relevant stakeholders to coordinate and ensure comprehensive care for residents.

Job Requirement :

• Proficient in English and able to converse in dialects or other languages
• Office Hours: 5 days work week
