Job summary

Agape Cp is seeking a dedicated Registered Nurse to assess clients’ health, care needs, and home environments across Singapore. The role involves creating and updating tailored care plans while ensuring safety and continuity of care.

You will supervise caregivers, coordinate deployments, and liaise with families and healthcare professionals. Travel across Singapore for home visits and urgent care support is required.

Qualifications

• Diploma or Degree in Nursing from a recognised institution.
• Registered Nurse with the Singapore Nursing Board, with a valid Practising Certificate.
• Preferably at least two years of clinical nursing experience; home care, community nursing or geriatric care is an advantage.

Responsibilities

• Assess clients’ health conditions, care needs, home environment and support requirements.
• Develop and review individualised care plans based on clients’ needs and condition changes.
• Manage assigned cases, conduct home visits and follow up with clients and families.
• Supervise caregivers, provide clinical guidance and ensure care is delivered safely.
• Coordinate with the operations team on caregiver matching, deployment, replacements and handovers.
• Communicate with clients, families and healthcare professionals to address concerns and maintain continuity of care.
• Respond to incidents and changes in clients’ conditions, ensuring escalation and follow-up.
• Participate in caregiver interviews, competency assessments, onboarding and training.
• Maintain accurate care plans, case notes, assessment records and incident reports while protecting client confidentiality.
• Support care quality reviews, infection prevention practices and service completion arrangements.

• Experience supervising or training caregivers or healthcare support staff is preferred.
• Strong communication and interpersonal skills, with the ability to work effectively with clients, families and care teams.
• Organised and able to manage multiple cases, prioritise tasks and maintain accurate documentation.
• Comfortable using digital care management systems and basic office software.
• Compassionate, patient and professional, with a commitment to client safety and confidentiality.
• Willing to travel across Singapore for home visits and provide support for urgent care matters when required.
