Job summary

AGAPE PLUS PTE. LTD. is seeking a Registered Nurse to assess clients’ health conditions, develop care plans, and manage home-based caseloads across Singapore. You will supervise caregivers, coordinate care handovers, and liaise with families and healthcare professionals to ensure safe, quality care.

The role requires a nursing degree, valid practicing certificate, and 2+ years of clinical/home-care experience. Travel across Singapore for home visits and urgent care support is expected.

Qualifications

• Diploma or Degree in Nursing from a recognised institution.
• Registered Nurse with the Singapore Nursing Board with a valid Practising Certificate.
• Preferably at least two years of clinical nursing experience; home care/geriatric/case management experience an advantage.
• Able to assess care needs, develop care plans and recognise changes needing escalation.
• Experience supervising or training caregivers or healthcare staff preferred.
• Strong communication and interpersonal skills with clients, families and care teams.
• Organised and able to manage multiple cases and maintain accurate documentation.
• Comfortable using digital care management systems and basic office software.
• Compassionate, patient and professional with a commitment to client safety and confidentiality.
• Willing to travel across Singapore for home visits and urgent care support.

Responsibilities

• Communicate with clients, families and healthcare professionals to address concerns and maintain continuity of care.
• Respond to incidents and changes in clients' conditions with escalation and follow-up.
• Participate in caregiver interviews, competency assessments, onboarding and training.

• Communicate with clients, families and healthcare professionals to address concerns and maintain continuity of care.
• Respond to incidents and changes in clients' conditions, ensuring appropriate escalation and follow-up.
• Participate in caregiver interviews, competency assessments, onboarding and training.
• Maintain accurate care plans, case notes, assessment records and incident reports while protecting client confidentiality.
• Support care quality reviews, audits, infection prevention practices and service completion arrangements.

Job Requirements

• Preferably at least two years of clinical nursing experience. Experience in home care, community nursing, geriatric care or case management is an advantage.
