ASEANA HEALTHCARE PTE LTD is looking for a responsible, organised and caring Care Coordinator to join our growing healthcare team.

The role focuses on coordinating ICS referrals, arranging start of care, and managing caregiver assignments, while maintaining regular communication with clients and hospitals to ensure seamless service delivery.

Qualifications

• Diploma or relevant experience in Healthcare, Nursing, Community Care, Social Services or Healthcare Administration.
• Experience in healthcare/home care coordination is an advantage.
• Good communication and interpersonal skills.
• Well-organised and able to manage multiple cases.
• Comfortable coordinating staff schedules and handling last-minute changes.
• Good computer and administrative skills.

Responsibilities

• Coordinate and manage ICS referrals from hospitals.
• Contact clients and families to arrange the start of care services.

• Arrange replacement staff when necessary.
