Singapore General Hospital is seeking an Assistant Manager to lead the development and implementation of a new initiative that connects hospital, primary, specialist and community care for integrated, person-centred care and seamless transitions to the community.

The Assistant Manager will help lead the development and implementation of a new initiative that includes collaborative care partnerships that connects hospital, primary, specialist and community care to enable integrated, person-centred care and seamless transitions to the community.
The role encompasses programme conceptualisation and planning; stakeholder engagement and hospital-wide coordination; development of protocols, procedures and workflows; project delivery, governance, communications and resource management; performance monitoring, evaluation and reporting; and recommendations to management.
In addition, the Assistant Manager is part of the administrative team that will support Division-wide initiatives and key committees in advancing the priorities of the Division of Population Health & Integrated Care.
This includes administrative and secretariat support, preparing management reports, presentations and submissions, proposal writing, budgeting exercises, and other administrative duties.
