Job summary

Changi General Hospital Pte Ltd is seeking a Wellbeing Coordinator to lead a multidisciplinary team in delivering integrated, person-centred health and social care within the SingHealth region. You will drive strategic decisions, manage partnerships, and shape care models to meet population needs.

The role requires strong leadership, excellent communication, and the ability to oversee budgets, performance metrics and staff development in a dynamic healthcare environment.

Qualifications

• Operational experience in health, social and community care sectors.
• Strong verbal and written communication skills.
• Minimum 4 years in leadership, managing teams and projects.
• Direct responsibility for major programme operations, budget or strategic direction.
• Advanced knowledge of case work, social work principles or relevant frameworks.
• Experience/accreditation in similar roles (health coach, social work) preferred.

Responsibilities

• Identify strengths/weaknesses of Wellbeing Coordinators’ role and design mitigation strategies.
• Support HOD in setting strategic vision and achieving targets and outcomes.
• Drive change management to develop new capabilities for evolving care needs.
• Establish strategic partnerships with internal and external stakeholders.
• Develop and review care models and operationalise them within the department.
• Lead interdisciplinary workgroups and influence patient care policies.
• Enhance awareness of Wellbeing Coordinators’ role within SingHealth and partners.
• Set care standards and ensure team compliance.
• Analyse data and supervise quality improvement projects and staff development.
• Monitor resource deployment and workload distribution for optimal use.
• Assist with department workplans and budget planning; ensure budget utilization.
• Generate monthly statistical reports; support staff growth and retention.
• Ensure WBC trainings and CPD for department; develop required competencies.
• Define competencies with stakeholders and develop trainings/lesson plans.
• Conduct orientation for new hires and demonstrable CPD activity.
• Guide management of complex cases and provide case escalation support.
• Provide care protocol development and periodic audits of care provision.
• Address gaps through case reviews and improve care delivery.
• Maintain high standards of care documentation and team communication.
• Ensure security and confidentiality of residents' or patients' data.
