Job summary

Singapore Health Services Pte Ltd (SingHealth HQ) invites experienced healthcare professionals to lead a dynamic Wellbeing Coordinators (WBCs) team focused on integrated community care across Singapore. You will oversee training and performance, manage highly complex cases, and develop plans addressing health and social needs for residents and caregivers.

Expect collaboration with partner agencies, mentorship of junior staff, and a commitment to continuous improvement.

Qualifications

• Bachelor's Degree in healthcare-related discipline with at least 6 years of experience.
• Health coaching or social work experience is preferred.
• Leadership experience in driving projects or supervising teams is preferred.
• Strong verbal and written communication skills are essential.
• Empathetic, able to relate to diverse individuals and tailor coaching methods.

Responsibilities

• Oversee Wellbeing Coordinators (WBCs), provide training and competency assessments, and monitor performance.
• Coordinate care and manage highly complex cases across multiple agencies.
• Develop intervention plans addressing health and social needs for clients and caregivers.
• Build partnerships with agencies and community partners to drive new programmes.
• Provide coaching/mentorship to junior WBCs and guide escalations.
• Lead quality improvement or research projects and contribute to service planning.
• Assist Head of Department with workplans, budgets, staffing, and recruitment.
• Occasionally participate in public speaking or community outreach.
