Qualifications

• Degree or Diploma with 8+ years relevant experience
• Experience in health/social/community care sectors, with basic screening tools, vital signs measurement and simple functional/fall-risk assessments
• Collaborative team player with strong verbal, written and interpersonal skills, and ability to coach and mentor junior Wellbeing Coordinators

Responsibilities

• Lead and mentor wellbeing staff to deliver person-centred care for residents with chronic and complex health and social needs across community and outreach settings
• Screen residents’ needs, coordinate, develop and implement holistic care plans with regular follow-ups
• Coach patients and caregivers in chronic disease self-management, fall prevention, healthy lifestyles, telecare use and financial support
• Ensure timely referrals, accurate documentation, and active participation in quality improvement
