Job summary

KK Women’s and Children’s Hospital Pte. Ltd. seeks an experienced Service Care Coordinator to handle case management for antenatal clients under the Home Visit Programme.

You will communicate effectively with patients and families, coordinate services, and provide administrative support to ensure smooth operations. Responsibilities include scheduling, IT issue troubleshooting, health and social screenings, and maintaining accurate records per organisational standards.

Qualifications

• Diploma with 2–4 years relevant experience, or N/O/N/A Level or NITEC in Healthcare with 6–10 years relevant experience.
• Must be able to communicate effectively, both orally and in writing.
• Must have records maintenance, organizing and coordinating skills.
• Strong word processing, receptionist and data entry skills.
• Ability to maintain calendars and schedule appointments.
• Confidential handling of records and information.
• Supervisory skills.

Responsibilities

• Coordinate service care and case management.
• Build rapport with antenatal patients, families and staff.
• Collaborate with interdepartmental and external colleagues to optimise health outcomes.
• Provide administrative and systems support for outreach and engagement.
• Assist in appointment scheduling and respond to client inquiries.
• Identify needs and screen patients; manage caseloads at clinics.
• Perform health and social screenings as part of antenatal care.
• Maintain documentation and reporting per organisational standards.
• Assist with other duties as assigned by Program Lead or Manager.
