Job summary

Diaverum AB is seeking a Pharmacist to lead pharmaceutical care management across assigned clinics, ensuring excellence in service delivery and adherence to clinical standards and governance.

You will coordinate drug therapy provision, participate in MDT discussions, and drive quality and preventive care initiatives to improve patient outcomes and wellbeing.

Qualifications

• Bachelor's degree in Doctor of Pharmacy.
• Certified Pharmacist Specialist by the Saudi Commission for Health Specialties (SCFHS).
• Fresh Graduate, 1-2 years clinical dietetics experience in a clinical setting or hospital is favorable.
• Experience in clinical specialty favorable (renal, diabetes, hypertension).
• Fluent in Arabic and English (spoken and written).
• Computer operating experience and MS Office skills are required.

Responsibilities

• Pharmacist is accountable towards the Direct Manager with regards to meeting clinic objectives including administrative business medical and operational outcomes.
• Pharmacist is accountable towards the Function Head with regards to ensuring standards of care and quality management.
• Work in collaboration with the MDT and patients to support patients to navigate their disease, dialysis treatment and required medication lifestyle changes.
• Initiate and take lead in managing pharmaceutical and preventive care projects in coordination and alignment with Head of Pharmaceutical Care and other strategic departmental and organizational projects as appropriate.
• Point of contact in the clinic for medical nursing operations and other functions with all matters concerning the pharmaceutical care of patients.
• Ensure effective drug therapy health promotion and lifestyle management supporting patients with different demographic background.
• Ensure implementation of a continuous accurate and effective pharmacotherapy plan for all patients.
• Manage the set clinical parameters to accurately measure monitor and evaluate all aspects of pharmaceutical care in accordance to national and international standards and guidelines.
• Utilize clinical knowledge and counselling skills to make a positive difference in patients quality of life.

Key Responsibilities

Definition of Reporting Lines

Pharmacist is accountable towards the Direct Manager with regards to meeting clinic objectives including administrative business medical and operational outcomes

Pharmacist is accountable towards the Function Head with regards to ensuring standards of care and quality management further specified below Annual performance evaluation including training and development needs is a shared responsibility between the Direct Manager and Function Head towards the Pharmacist

Role within the Multidisciplinary Team

MDT Aim to cultivate pharmaceutical care that is patient-centric evidence-based and integrated within Diaverum care delivery system Work in collaboration with the MDT and patients to support patients to navigate their disease disease complications dialysis treatment and required medication lifestyle changes Initiate and take lead in managing pharmaceutical and preventive care projects in coordination and alignment with Head of Pharmaceutical Care and other strategic departmental and organizational projects as appropriate Point of contact in the clinic for medical nursing operations and other functions with all matters concerning the pharmaceutical care of patients

Pharmaceutical Medical Clinical Performance

• Ensure effective drug therapy health promotion and lifestyle management supporting patients with different demographic background gender age educational level medical history and psychosocial status
• Ensure implementation of a continuous accurate and effective pharmacotherapy plan for all patients includes screening assessment diagnosis intervention reconciliation documentation monitoring and evaluation
• Manage the set clinical parameters to accurately measure monitor and evaluate all aspects of pharmaceutical care in accordance to national and international standards and guidelines in renal care and chronic disease management
• Utilize clinical knowledge and counselling skills to make a positive difference in patients quality of life
• Utilize clinical knowledge and counselling skills to make a positive difference in patients medical management
• Engage and actively participate in dietetic research led by the pharmaceutical care department

Pharmaceutical Supplies

• Manage Pharmaceutical Supplies with ensure both Medications Safety Product Quality in terms of shipping delivery storage and preparation availability of the product to cover any situation of need
• Coordinate with Logistics Services regarding quality and availability submit accurate reports to determine the need continuously - submit accurate reports of critical pharmaceutical items ahead of time to SC
• Review all pharmaceutical stock data that like slow moving item over stock items nearly expired items and zero stock items etc and give recommendations
• Study the drugs statistical reports to develop appropriate solutions for any deficit
• Ensure following medication manufacturing recommendations in all medications management steps
• Make necessary consumption analysis to improve clinic consumption take patients need into consecration at all times
• Coordinate with MDT Logistics Services team to prevent the waste of pharmaceutical supplies
• Stay abreast of the latest updates in the pharmaceutical industry to obtain the best quality products as the available capabilities allow
• Follow all pharmaceutical supplies P and P accordingly

Policies and Procedures

• Understand and comply with policies and procedures related to scope of care patient confidentiality rights infection control facility management and safety as well adhering to time attendance and dress code requirements
• Understand policies and procedures and apply them to meet patient s need as determined by the medical staff prescription order
• Adhere to current policies and procedures and future updates in Diaverum including quality standards such as Joint Commission International JCI and or Saudi Central Board for Accreditation of Healthcare Institutions CBAHI medical KPIs as well as all local regulatory requirements governing nutrition care
• Monitor compliance according to all pharmaceutical care policies and procedures and country regulations

Quality Management

• Work closely with Head of Pharmaceutical Care Clinic Medical Director Clinic Team and Operation managements in achieving overall quality goals and medical parameters
• Participate in Pharmaceutical care department meetings clinic management meetings including quarterly management review QMR and monthly business review meetings MBR
• Participate in country and clinic development initiatives projects while ensuring alignment with Diaverum Pharmaceutical care department strategy and vision

Pharmacist Learning and Development

• Participate and engage in all provided training programs and continuous professional development of pharmaceutical care
• Participate in the orientation and induction of new members of pharmaceutical team as applicable
• Participate in self and other s education training and development including assigned pharmacist interns as applicable
