The role centers on reviewing inpatient records, identifying documentation gaps, and generating compliant physician queries to enable accurate coding and quality reporting.

Qualifications

• Minimum four (4) years of bedside nursing experience, or at least three (3) years in ICU/ED/OR; UR or CDI exposure.
• Experience using electronic health record (EHR) systems to review clinical documentation.
• Experience developing physician queries and resolving documentation gaps.
• Strong knowledge of ICD-10-CM/PCS, DRG, SOI, ROM, CCs and MCCs.

Responsibilities

What is the job?

The Virtual Nurse (RN) is responsible in reviewing inpatient medical records to ensure clinical documentation accurately reflects the patient's diagnoses, treatment, severity, and risk of mortality. The role identifies documentation gaps, develops compliant physician queries, and collaborates with clinical and coding teams to support accurate coding, quality reporting, and reimbursement.

• Review inpatient medical records for accuracy, completeness, and clinical consistency.
• Identify missing, conflicting, or unclear clinical documentation.
• Develop compliant physician queries to clarify clinical documentation.
• Evaluate diagnoses, procedures, severity of illness (SOI), risk of mortality (ROM), complications, and comorbidities.
• Support accurate ICD-10-CM/PCS coding and DRG assignment through clinically supported documentation.
• Collaborate with physicians, coders, and clinical teams to resolve documentation questions.
• Monitor CDI quality, productivity, and query performance metrics.
• Participate in audits and quality improvement activities.
• Provide guidance and mentoring to other CDI specialists as needed.
• Follow applicable AHIMA, ACDIS, CMS, and organizational CDI guidelines.

Required Qualifications:

Minimum of four (4) years of bedside nursing experience or at least three (3) years of experience in ICU, ED, or OR exposure, with awareness of utilization review (UR) or CDI.

Experience using electronic health record (EHR) systems to review clinical documentation.

Experience developing physician queries and resolving clinical documentation gaps.
