Job summary

Bon Secours Mercy Health in the Philippines seeks a Transfer Center Access Expert to coordinate patient transfer logistics, bed placement, and continuity of care across facilities. You will work with clinicians to ensure appropriate level of care and timely patient placement, while accurately documenting in EMR and protecting PHI.

Requires PHRN and US RN licenses, a Bachelor’s in Nursing, and 3+ years healthcare experience. Epic EHR and Genesys experience are preferred.

Qualifications

• Active PH registered nurse license (PHRN) required
• US Registered Nurse license required
• Bachelor’s Degree in Nursing or related healthcare field required
• 3+ years healthcare or healthcare contact center experience required
• 1+ year clinical environment experience required
• 1+ year experience in transfer center or US hospital preferred
• Epic EHR and Genesys experience preferred

Responsibilities

• Serve as first-line clinical and operational resource for TransferCenterAccess team
• Coordinate patient intake, transfer, and access workflows
• Collaborate with physicians, nurses and care teams across facilities
• Facilitate care coordination activities including transportation arrangements and scheduling
• Document accurately in EMR and protect PHI
• Maintain awareness of case status and escalation needs
• Support regulatory and quality initiatives and reporting
