Bon Secours Mercy Health is seeking a Transfer Center Access Specialist in the Philippines who coordinates patient transfer logistics, bed placement, and care continuity across facilities. You will support intake workflows, communications with clinicians, and transportation arrangements to ensure timely patient care.

Primary Function/General Purpose of Position
The TransferCenterAccessSpecialist coordinates patient transferlogisticsand supports patient acceptance, bed placement, and continuity of care using client-defined workflows to ensure patients receive care at theappropriate leveland capability. This role collaborates closely with clinical teams, physicians, leadership, and referring facilities to facilitate efficient patientlogistics,identifyand report barriers to access and throughput, and uphold professional, clinical, and evidence-based standards. Responsibilities also include providing non-licensed clinical and clerical support such as call intake, care coordination, scheduling, and communication to support patient logistics and care team collaboration across assigned services or books of business.
Essential Job Functions
Coordinatefunctionsacrossintakeservices,patientaccessand transferworkflows to identify appropriatelevelof care, accepting providers, consultations, and bed placement across all referral sources, with a focus on quality, safety, and efficiency.
Collaborate directly with physicians, nurses, and care teams at referring and receiving facilities to ensureaccurateclinical communication,timelyacceptance,appropriate bedassignment, and development of an effective plan of care.
Serve as a liaison between patients, caregivers, providers, hospital staff, and external agencies through professional and effective communication whilefacilitatingcare coordination activities—including transportation arrangements, scheduling, care instructions, escalation, and follow-up—in accordance withestablished workflows, algorithms, and delegated responsibilities.
Apply critical thinking, clinicalexperience, and problem-solving skills to recommendoptimalpatient outcomes based on clinical information, preferences, and facility capability and capacity.
Utilize established workflows and algorithms todetermineintake completion, escalation needs, care team activation, and ongoing case status management.
Accurately document, enter, retrieve, and protect patient information within theElectronic Medical Record (EMR)and all required systems, ensuring compliance with regulatory, privacy, and data collection standards.
DemonstrateproficiencywithElectronic Health Record (EHR)platforms, telephony, and office systems whilemaintainingdata accuracy, timeliness, and protection of PHI.
Maintain awareness of care events and case status byfacilitatingongoing communication related to instructions, escalation, bed assignment, transportation, readmission potential, discharge planning, andfollow‑upneeds.
Monitor individual and team performance and compliance, support regulatory and quality initiatives, andidentify, report, and recommend solutions for safety, compliance, and operational risks.
Demonstrate knowledge of regulatory requirements including EMTALA, HIPAA, ethics, compliance, and quality standards, and accelerate concerns through appropriate chainof command.
Maintain professional, collaborative relationships with interdisciplinary care teams andparticipatein shared leadership.
Participate in process improvement, professional development, peer development, and peer review activities, while performingadditionaltasks, projects, and analyses as assigned or directed.

Education
