Job summary

AGS Health Philippines, Inc. is seeking a Team Leader for Plan of Care Coordination (USRN) in Taguig. The role oversees daily operations of the Plan of Care Review team, ensuring high-quality case reviews, coaching, and performance management while maintaining patient care standards.

The successful candidate will have US RN licensure, 3+ years clinical experience in relevant areas, and at least 1 year of supervisory experience. Strong EMR and MS Office skills are essential for success.

Qualifications

Active US RN license is required.
3+ years clinical experience in home health, utilization management, care coordination, or related reviews.
1+ year of supervisory or team leadership experience.
Strong understanding of Plans of Care (Form 485), recertification, and resumption-of-care processes.
Excellent written and verbal communication skills.

Responsibilities

Provide day-to-day leadership and support to Plan of Care Review team.
Monitor productivity, quality, attendance, and service levels.
Manage workload distribution and resources for timely case completion.
Serve as first escalation point for clinical, workflow, and operational concerns.
Conduct regular team meetings, coaching, and performance reviews.
Foster accountability, collaboration, and continuous improvement.
