AGS Health Philippines, Inc is seeking a Team Leader - Plan of Care Coordination (USRN) to oversee the Plan of Care Review team on-site in McKinley, Taguig. The role emphasizes clinical leadership, quality assurance, and continuous improvement in a fast-paced healthcare environment.

The incumbent will coach, train, and evaluate staff, manage workload and productivity, and act as the escalation point for clinical and operational issues while ensuring compliance with workflows and standards.

Qualifications

• Active US Registered Nurse license is required.
• Minimum 3 years of clinical experience in home health, utilization management, care coordination, or related clinical review.
• Minimum 1 year of supervisory, coaching, mentoring, or team leadership experience.
• Strong knowledge of home health documentation (Plan of Care 485), recertification, and resumption-of-care processes.
• Excellent written and verbal communication skills.
• Strong analytical, organizational, and problem-solving abilities.

Responsibilities

• Provide day-to-day leadership and support to the Plan of Care Review team.
• Monitor productivity, quality, attendance, and service levels.
• Manage workload distribution and resource allocation for timely case completion.
• Escalate clinical, workflow, and operational concerns as first point of contact.
• Conduct regular team meetings, coaching sessions, and performance feedback.
• Collaborate with recruitment, onboarding, and integration of new members.
