Job summary

AGS Health Philippines, Inc. seeks US Registered Nurses (USRN) to join our Home Health Care Plan team onsite in Taguig. You will review records, develop care plans, and support patient-centered goals for US clients.

The ideal candidate holds an active PHRN/USRN license (NCLEX-RN passer) with strong bedside experience, sharp clinical judgment, and solid care coordination skills. Experience in home health or managed care is a plus.

Qualifications

• Active PHRN / USRN license (NCLEX-RN passer).
• Strong bedside nursing experience with sound clinical judgment.
• Ability to assess, plan, and develop home health care plans.
• Excellent critical thinking and care coordination skills.
• Experience in home health, case management, or managed care is an advantage.
• Willingness to work onsite at Cyber Sigma, MCK West Taguig.

Responsibilities

• Review medical records and patient information.
• Create and update individualized home health care plans.
• Ensure diagnoses, medications, therapy goals, and safety measures are accurate.
• Identify missing information and coordinate with teams to obtain corrections.
• Support high-quality patient care by maintaining complete documentation.
• Help reduce hospital readmissions through proper care planning and follow-up.
