Job Title: Team Leader - Plan of Care Coordination (USRN)

Position Summary
The Team Leader is responsible for overseeing the daily operations, quality, productivity, and development of the Plan of Care Coordination team. This role serves as the primary point of contact for clinical and operational escalations, ensures compliance with established workflows and quality standards, and supports continuous improvement initiatives. The Team Leader will also perform case reviews as needed while providing coaching, training, and performance management to team members.
The Team Leader must possess strong clinical knowledge, leadership skills, and the ability to effectively manage a team in a fast-paced healthcare environment.
Minimum Requirements
- Active US Registered Nurse (RN) license.
- Minimum of 3 years of clinical experience, preferably in home health, utilization management, care coordination, or related clinical review functions.
- Minimum of 1 year of supervisory, coaching, mentoring, or team leadership experience.
- Strong understanding of home health documentation, Plans of Care (Form 485), recertification, and resumption-of-care processes.
- Excellent written and verbal communication skills.
- Strong analytical, organizational, and problem-solving skills.
- Ability to work independently and lead a team effectively.
- Proficient in EMR systems and Microsoft Office applications.
- Willing to work assigned Manila day or evening shift schedules.
Key Responsibilities
Team Leadership & Operations
- Provide day-to-day leadership and support to the Plan of Care Review team.
- Monitor daily productivity, quality, attendance, and service level performance.
- Manage workload distribution and resource allocation to ensure timely completion of assigned cases.
- Serve as the first point of escalation for clinical, workflow, and operational concerns.
- Conduct regular team meetings, huddles, and one-on-one coaching sessions.
- Foster a culture of accountability, collaboration, and continuous improvement.
- Support recruitment, onboarding, and integration of new team members.
Quality Management
- Ensure team compliance with policies, procedures, and clinical standards.
- Review completed cases for accuracy, completeness, and adherence to established workflows.
- Conduct routine audits and quality reviews.
- Identify quality trends and implement corrective action plans.
- Provide coaching and feedback to improve individual and team performance.
- Partner with stakeholders on quality improvement initiatives.
Training & Development
- Deliver ongoing training, refresher sessions, and skills development programs.
- Act as a subject matter expert (SME) for Plan of Care Review and Recertification processes.
- Support knowledge transfer and train-the-trainer activities.
- Maintain training documentation and monitor competency requirements.
- Ensure team readiness for process updates and workflow changes.
Client & Stakeholder Management
- Collaborate with clinical and operational leadership.
- Communicate process updates, issues, and performance results to stakeholders.
- Participate in calibration sessions, audits, and client meetings as required.
- Escalate workflow, documentation, and system issues impacting performance or quality.
