Job summary

Limitlessli is seeking a Home Health Care Coordinator to manage and coordinate remote home health services for US clients. You will act as the central contact among patients, clinicians, and referral sources, ensuring timely starts of care and adherence to CMS guidelines.

The role requires a US RN license in New York and at least 2 years of home health or case management experience. Strong documentation, communication, and scheduling skills are essential for success in a remote, globally

Qualifications

Must be a US Registered Nurse with New York license.
Minimum 2 years of experience in home health care, case management, or clinical coordination.
Knowledge of Medicare Conditions of Participation, OASIS documentation, and HIPAA compliance required.

Responsibilities

Intake and Referral Management: receive and process new referrals, verify eligibility and required documentation, obtain physician orders, coordinate start-of-care visits.
Care Coordination and Case Management: liaise between patients, families, physicians, clinicians, and resources; ensure plans of care meet CMS guidelines and orders; monitor visit frequencies and outcomes.
Documentation and Compliance: ensure timely entry of patient data into the EHR; maintain Medicare documentation requirements; participate in QAPI activities; assist with audits and surveys.
Scheduling and Staff Coordination: schedule clinicians based on patient needs and staff availability; communicate changes promptly; help ensure adequate staffing.
Communication and Customer Service: provide excellent service to patients, families, referral sources, and staff; resolve concerns promptly; promote the agency image.
