Job summary

Seamlessassist seeks a Cardiology Patient Navigator (Coordinator) to support a U.S.-based cardiology team overnight and on weekends. The role centers on patient communication, scheduling, care coordination, remote monitoring, and clinical documentation in a remote setting.

The ideal candidate has an active nursing license (RN preferred), 3+ years in healthcare or telehealth, strong English, and familiarity with EMR/EHR and RPM dashboards.

Qualifications

• Active nursing license is required (RN preferred).
• 3+ years in healthcare, telehealth, or medical office settings.
• Experience in care coordination or care management, preferably telephonic/remote.
• Fluent English (B2+) and strong written/verbal communication.
• Proficient with EMR/EHR systems and remote monitoring dashboards.
• Demonstrated AI fluency and ability to use AI tools to work efficiently.

Responsibilities

• Manage patient communication, scheduling, reminders, and follow-up workflows in EMR/EHR systems.
• Conduct telephonic/remote outreach to reinforce care instructions and coordinate next steps.
• Support cardiology care pathways and CHF management protocols.
• Monitor RPM alerts for weight and blood pressure; escalate concerns promptly.
• Maintain HIPAA-compliant documentation and coordinate billing/insurance as needed.
