Job summary

Mission Hospice and Home Care in San Mateo, CA, is seeking a per diem Hospice Nurse Case Manager to provide skilled nursing assessment, planning and care to maximize comfort for patients and families.

The role covers San Mateo County with additional coverage to Santa Clara County; schedule is Sunday, Thursday and Friday 8:30am–5:00pm; California RN license required with home health or hospice experience preferred.

Qualifications

• One year experience as a professional nurse in the past three years or a BSN.
• Experience in home health or hospice setting preferred.
• Current license as a registered professional nurse in California.

Responsibilities

• Assess, plan, implement and evaluate patient/family needs using the nursing process.
• Coordinate patient/family services with the transdisciplinary team.
• Use a case management approach and refer to other services as needed.
• Coordinate the plan of care and ensure care coordination for each patient.
• Apply admission and re-certification criteria to hospice care to establish levels of care.
