Qualificações

• Associates or Bachelor’s degree preferred, or equivalent combination of education and experience.
• Prior experience in healthcare, customer service or similar roles preferred.
• Proficiency with systems including electronic health records (EHR), Microsoft, Adobe, and Google Suite.
• Excellent communication skills, both written and verbal.
• Strong attention to detail.
• Strong organizational and time management skills.
• Ability to work both independently and collaboratively in a fast-paced environment.
• Unwavering commitment to patient-centered care and professional ethics.

Responsabilidades

• Respond to patient and provider support requests and communications via in-person, phone or electronic channels.
• Collaborate with care team members to prioritize quality and integrated care.
• Match new and current patient service requests based on set criteria (reason for treatment, insurance, provider availability).
• Monitor documentation completion to ensure regulatory compliance.
• Handle scheduling, records requests, releases of information, and information updates.
• Support other care coordinators to deliver patient-centered, trauma-informed care.
• Provide data and qualitative feedback to improve organizational processes.
