CMHA-YRSS is hiring a Case Manager in its Housing First program, based out of the Aurora office with travel across York Region & South Simcoe. This full-time role focuses on housing stabilization, recovery, and community integration for clients challenged by homelessness and mental health barriers.

Qualifikationen

• Degree with four years’ experience or Diploma with six years’ experience in a community-based mental health organization.
• Current registration with OCSWSSW or a regulatory body under the Regulated Health Professions Act; cannot be in the qualifying stages.
• Second language (French, Cantonese, Farsi, Italian, Korean, Mandarin, Punjabi, Russian, Tamil, Urdu) is an asset.
• Knowledge of the Ontario Mental Health Act, Residency Tenant Act, mental health reform principles and related acts.
• Ability to apply Recovery principles and empowerment-oriented philosophies in work with consumers.
• Extensive knowledge of supports and services in York Region and South Simcoe.
• Excellent communication skills and ability to work collaboratively.

Aufgaben

• Adheres to Intensive Case Management Services Standards of the Ontario Ministry of Health and Long-Term Care.
• Maintains a client caseload in accordance with the standards.
• Responds to trauma signs and provides referrals and short-term assistance.
• Initiates referrals to other CMHA programs or external agencies as required.
• Anticipates and responds to the needs of internal and external clients.
• Establishes collaborative partnerships with clients using empowerment/Recovery principles.
