Job summary

(CHR) Community Health Resources, Inc. is seeking a Recovery Support Specialist for the Manchester, CT program. This part-time role supports clients using their lived recovery experience and coordinates with hospital staff, EMS, and the client’s support network.

The position emphasizes advocacy, recovery goal setting, and connecting clients with community resources and follow-up care, with opportunities for professional development and ongoing supervision.

Qualifications

• Education: Bachelor’s degree preferred or equivalent
• Recovery Coach Certification CCAR or equivalent
• Certified Peer Support & Recovery Professional or related credential

Responsibilities

• Ability to work with hospital-based staff, EMS, law enforcement, and client’s natural supports.
• Using lived recovery experience, assist clients in identifying and advocating their own recovery goals and inform others on recovery strategies.
• May provide independent and/or co-response with mobile clinical staff on crisis outreaches and follow up visits to promote client-centered, recovery-based approaches.
• Participates in interventions to help reduce health care disparities, including screening for Social Determinants of Health (SDOH).
• Complies and shares knowledge of community resources and links clients to behavioral health follow up care and support networks.
