Job summary

North Bay Regional Center is seeking a Social Services Case Manager to oversee service delivery for regional center clients, including those with complex needs. You will ensure Individual Program Plans are implemented and coordinated with providers, families, and team members.

Responsibilities include person-centered planning, vendor monitoring, and annual reassessments. A Bachelor’s is required; a Master’s is preferred, with at least 3 years’ relevant experience.

Qualifications

• Bachelor’s degree in a field related to developmental disabilities.
• Master’s degree preferred.
• 3 years’ experience with individuals with developmental disabilities.
• Bilingual ability (Spanish to English) preferred.

Responsibilities

• Manage the delivery of services to NBRC clients, including those with intensive needs.
• Ensure client IPP objectives are met and services are appropriately coordinated.
• Monitor vendor services and verify adherence to service authorizations.
• Evaluate IPP effectiveness and arrange access to community resources.
• Conduct annual reassessments with the interdisciplinary team; occasional client assessments.
