Zusammenfassung

CMHA York Region is hiring a Housing Support Worker for the Community Homes for Opportunity (CHO) Program in Aurora. This role focuses on helping adults with serious mental illness maintain housing and integrate into the community.

The position is a 1-year contract, full-time (35 hours/week) with evening/weekend hours as required. Salary range CAD 56,444–68,429 per annum; starting salary may reflect experience.

Qualifikationen

• A degree or diploma in health or social services.
• Current Registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
• Degree plus four years or more experience in a community-based organization working with adults with serious mental illness in the community.
• Diploma plus six years or more experience in a community-based organization working with adults with serious mental illness in the community.
• Experience working with people in congregate living environments, Homes for Special Care or Housing with Supports is preferred.
• Experience with individuals experiencing serious mental illness, concurrent disorders and dual diagnosis is preferred.
• Knowledge of the Ontario Mental Health Act, mental health reform principles, the Substitute Decisions Act, the Health Care Consent Act and PHIPA requirements.
• Ability to apply Recovery principles and empowerment oriented philosophies and practices in work with clients.
• A second language reflecting the local community is preferred.

Aufgaben

• Help clients maintain their housing and engage with the community.
• Budgeting and financial management support and facilitating access to resources.
• Develop and implement wellness plans and action steps.
• Collaborate with CHO Housing Administrators and Home Operators as part of a multi‑disciplinary team.
• Advocate for clients and build relationships to improve resource access.
