Job summary

CHR is seeking a Recovery Support Specialist for the MH Emergency Department in Manchester, CT. This full-time role requires travel between hospitals and a 40-hour work week, Mon-Fri 10:30am–7pm.

You will help clients identify recovery goals, use motivational interventions, conduct assessments, and collaborate on individualized treatment plans. The role includes crisis support, case management, SDOH screening, and connection to community resources.

Qualifications

• Lived experience with recovery from mental health or substance use is required.
• 2+ years working with identified populations; RSS preferred.

Responsibilities

• Assist clients in identifying and advocating recovery goals.
• Inform and educate on recovery strategies using motivational interventions.
• Conduct program assessments and screenings as indicated.
• Collaborate to create individualized, person-centered treatment plans.
• Provide skills teaching and crisis intervention as needed.
• Provide case management and connect to supports; coordinate with treatment teams.
• Screen for Social Determinants of Health (SDOH) and address barriers to care.
• Link clients to behavioral health follow-up and community resources.
• Provide follow-up recovery coaching and safety planning; involve family where appropriate.
• Maintain documentation per program standards.
• Attend staff meetings and trainings; transport clients as required.
