Job summary

Snapscale is seeking a reliable Healthcare Virtual Assistant (HVA) to support a US-based primary and urgent care network as a Case Manager. You will handle patient outreach, scheduling, care coordination, and obtaining medical records to support care quality metrics.

Key responsibilities include coordinating care between providers and labs, managing patient inquiries, and maintaining EMR documentation. Must be able to work onsite in Davao and communicate effectively in English.

Qualifications

• Experience as Healthcare Virtual Assistant or similar healthcare admin role.
• Proven patient outreach and care coordination capabilities.
• Strong attention to detail and administrative follow-through.
• Excellent written and verbal English communication.
• Experience with EMR/EHR systems (e.g., AdvancedMD) and VoIP.

Responsibilities

• Call patients to schedule appointments and follow-up visits.
• Perform patient outreach to confirm completion of health screenings.
• Identify facilities where screenings were performed.
• Request, follow up on, and obtain medical records for screenings.
• Coordinate care between providers, labs, and patients.
• Assist with patient intake, registration, and inquiries.
• Update and maintain patient records within the EMR system.
• Support quality programs and health outcome workflows (ACOs, HEDIS).
• Provide additional healthcare administrative support as assigned.
