E-Teleconnect, Inc. in Parañaque City is seeking an Afterhours Intake Coordinator to support the onboarding of patients into Supportive Home Health. This non-bedside, operations-focused role handles referrals, documentation, and initial patient contact to ensure timely transitions from hospital to home health services.

The position emphasizes documentation management, intake triage, and collaboration with Case Management and Scheduling teams to ensure accurate and complete patient records.

Qualifications

• Minimum 2 years in case management, utilization management, or intake coordination.
• Experience with Medicare/Medicaid home health requirements preferred.
• Healthcare operations or healthcare BPO experience is a plus.

Responsibilities

• Receive and manage incoming referrals via fax, email, and chat platforms.
• Review referral packets for completeness including H&P, orders, discharge notes, authorizations, and supporting clinical documents.
• Organize and upload all documentation into the EMR system.
• Ensure documentation is complete and ready for Case Management endorsement.
• Conduct initial clinical review of referral documentation to determine eligibility for home health services.
• Identify missing documentation or inconsistencies and escalate accordingly.
• Apply basic clinical judgment to determine if the patient is appropriate for home health admission.
• Conduct initial outreach to patients to confirm referral to Supportive Home Health.
• Provide overview of services (RN, LVN, PT, OT, ST as stated on physician orders).
• Set expectations for follow-up calls and home visits.
• Coordinate handoff to Case Management and Scheduling teams.
• Accurately input patient demographic and clinical data into EMR/EHR systems.
• Track referral status and pending documentation.
• Maintain timely updates on intake progress and readiness for admission.
