Job summary

Access Community Health is seeking an Advanced Support Worker to work with care coordination and district nurses, performing complex client care and evaluations under supervision. You will lead training for newer staff and contribute to a collaborative, multi-disciplinary team in a home-based care setting across Aotearoa.

Ideal candidates have at least 12 months in a support worker role, a NZ driver's license, and a Level 4 Health and Wellbeing certificate.

Qualifications

• Must have 12 months’ proven experience in a Support Worker role.
• Must have a full or restricted New Zealand driver's license.
• Level 4 Health and Wellbeing Certificate (or equivalent).
• Experience in behaviour management is preferred but not mandatory.

Responsibilities

• Conduct non-complex client evaluations as directed by the Care Coordinator Team Leader.
• Provide complex client care and carry out advanced tasks with competency verified by the clinical line manager.
• Take on a leadership role, delivering on-the-job and just-in-time training to less experienced staff.
