Access Community Health is seeking an Advanced Support Worker to work with care coordination and district nurses, performing complex client care and evaluations under supervision. The role involves conducting non-complex client evaluations, as directed by the local Care Coordinator Team Leader.
You will also provide complex client care and carry out advanced tasks, for which you will have received training and competency verification from your clinical line manager. Additionally, you will take on a leadership role, offering on-the-job and just-in-time training to less experienced support workers, helping to develop and strengthen the SW workforce.
