Job summary

Te Whatu Ora Waitematā invites an experienced Needs Assessor to join the NASC team in Waitematā. This permanent full-time role focuses on assessment and coordination of support services for older adults and younger people with chronic health needs, to enable independent living at home or a smooth transition to Residential Care when required.

The ideal candidate will hold NZ registration (Allied Health or Nursing) with an annual practicing certificate and a valid full driver’s license.

Qualifications

  • Current registration as an Allied Health professional or Nurse in New Zealand.
  • Full NZ annual practicing certificate.
  • Valid full driver’s license.
  • Experience in assessment and service coordination.

Responsibilities

  • Assess and coordinate services for older adults or younger individuals with chronic conditions.
  • Collaborate with clients and whānau to tailor care packages to support independence at home or safe transitions to residential care.
  • Utilise strong communication and negotiation skills to build positive relationships with clients and stakeholders.

Needs Assessment and Service Coordination (NASC) Team

Permanent Full time FTE

An exciting opportunity has arisen in the NASC team Waitematā. We have a permanent full-time opportunity in the community.

Join NASC in our mission to provide assessment and coordination of support services for older adults with health and disability-related needs, as well as for younger individuals managing chronic long-term health conditions. Our goal is to facilitate independent living at home or assist with a smooth transition to Residential Care when necessary.

We are actively seeking an experienced needs assessor whose professional background is in Allied Health or Nursing to join our team. The ideal candidate will possess current professional registration and a New Zealand annual practicing certificate, along with a valid full driver's license. Demonstrated expertise in assessment and service coordination is essential.

Successful applicants will have a track record of working collaboratively with clients and their whanau to tailor care packages that align with their support requirements. Exceptional communication and negotiation skills, coupled with a history of positive relationship building, are essential.
