Job summary

BlueCare is seeking a Care Partner to deliver person-centred, high-quality care in the Hamilton region, working with clinical teams to develop and review care plans and coordinate services for clients funded through aged care programs.

You will manage budgets, liaise with care teams, and monitor outcomes while spending time on the road engaging with clients in the community. The role requires a Certificate IV in a Health or Community Service discipline and a valid driver’s licence.

Qualifications

• Demonstrated understanding of client-centred, community-based care services delivery.
• Ability to liaise with clinical and care teams to provide coordinated care.

Responsibilities

• Develop and review care plans with clients and multidisciplinary teams.
• Manage budgets and facilitate multidisciplinary care activities.
• Liaise with clinical teams to monitor changing needs of complex community clients.
• Continuously monitor, review and evaluate care plans, services and outcomes.
• Assist clients in navigating the care system and linking to resources.
