Job summary

Taranaki District Health Board is seeking a Care Manager within the Community Services Group to work with older people, palliative clients and those with chronic conditions across home, hospital and community settings.

The role involves comprehensive assessments, developing person-centred care plans with whānau, coordinating support packages and regular reviews, with opportunities for some remote work and a NZ driver’s licence essential.

Qualifications

• Hold a professional qualification and NZ APC as part of scope of practice.
• Experience with older people, palliative care, or chronic conditions is preferred.
• Strong written and verbal communication skills are essential.
• Knowledge of assessment and care planning processes and interRAI assessments is advantageous.

Responsibilities

• Conduct comprehensive assessments and develop person-centred care plans with whānau.
• Coordinate and review care packages to meet individual needs.
• Collaborate with hospital clinicians, community services and other providers.
