Job summary

BayCare Health System in Dunedin, FL seeks a Discharge Planner Pool to join the Behavioral Health team at Mease Dunedin. The role involves assessing and planning for discharge, coordinating with community resources, and supporting patients in acute mental health and substance use treatment.

Requires BLS certification and relevant behavioral health and acute care experience. This pool position offers weekend rotation and travel may be required.

Qualifications

• BLS (Basic Life Support) certification is required.
• Behavioral Health experience: 1 year.
• Child and Adolescent experience: 2 years (SIPP Only).
• Acute Care experience may be required or preferred.

Responsibilities

• Provides assessment, linkage, monitoring, planning and advocacy for adults and/or children in acute mental health or substance abuse care.
• Understands discharge and transition planning and links with community resources.
• Oversees legal status issues related to involuntary admissions or Baker Act regulations.
• Acts as an active member of the treatment team and provides support.
• Other duties as assigned by supervisor.

Education

Bachelor's degree in Counseling/Health Education/Related Field/Social Work/Human Services/Psychology/Nursing

• Facility: Mease Dunedin- Behavioral Health
• Location: Dunedin, FL
• Status: Pool, Hourly
• Shift Hours: 8:00 am-4:30 am
• Weekend Work: 4 Weekend Days/ Month
• On Call: No

The Discharge Planner Pool Responsibilities

• Provides assessment, linkage, monitoring, planning and advocacy for adult and or children experiencing acute mental health and or substance abuse issues in an acute care inpatient setting.
• Possesses a clear understanding of discharge and transition planning and linkage with community resources.
• Oversees legal status issues related to the individuals involuntary or in accordance with the Baker Act regulations.
• Provides support as active member of the treatment team.
• Will perform other duties as assigned by supervisor.

Travel may be required.

Required
