Job summary

Comcare Trust is seeking a Complex Case Coordinator to support people with complex needs as they move from hospital or residential settings toward independent living. You will work within the Community Integration Service, applying Te Whare Tapa Wha and strengths-based planning.

The role requires mental health experience, collaborative planning, and strong documentation skills. A NZ Driver’s license and NZ working rights are essential, with a mission to foster empowerment and community inclusion.

Qualifications

• Strong communication and interpersonal skills.
• High literacy for planning and documentation.
• Problem solving with a solutions-focused approach.

Responsibilities

• Collaborate with clients through transition from hospital/residential to independent living.
• Implement transition plans with involved workers (e.g., Social Workers, GP, MSD, family).
• Maintain records in the PMS system per Comcare procedures.
