Job summary

Health New Zealand | Te Whatu Ora is seeking a Needs Assessor/Service Coordinator to assess the needs and goals of people aged 65+ living with long-term health or disability conditions. You will perform InterRAI Home Care assessments with clients, families and whānau, identifying supports to keep them safe, independent and engaged in their communities.

The role spans community and inpatient settings, requires clinical judgement to coordinate services, and involves travel around the Bay of

Qualifications

• Must hold current Annual Practising Certificate as a health professional.
• Full NZ driver’s licence and willingness to travel for client visits.
• Strong communication and interpersonal skills; capable of working with diverse communities.

Responsibilities

• Conduct InterRAI Home Care assessments for 65+ clients.
• Assess needs, goals and resources with clients, whānau and providers.
• Coordinate services and connect clients with funded supports.
