Comcare Trust is seeking a Complex Case Coordinator to support people with complex needs as they move from hospital or residential settings toward independent living. You will work within the Community Integration Service, applying Te Whare Tapa Wha and strengths-based planning.
The role requires mental health experience, collaborative planning, and strong documentation skills. A NZ Driver’s license and NZ working rights are essential, with a mission to foster empowerment and community inclusion.

We are looking for a Complex Case Coordinator to join our Community Integration Service.
The Community Integration Service works with people who have complex social or other issues that create barriers for successful community living and supports them to problem-solve a pathway forward to independence.
The successful person will have experience in the mental health arena and have skills in collaborative planning for clients with complex needs, thinking outside the square and developing service provision based on Te Whare Tapa Wha and strengths models.
To be successful in this role, you will have:
You must have a New Zealand Certificate in Health and Wellbeing Level 4 - Mental Health and Addiction Support (or equivalent qualification).
Experience in the mental health and disability sector would be an advantage.
Applicants must hold a current full, clean driver’s license.
You will have a cultural awareness, knowledge of, and demonstrable commitment to the principles of Te Tiriti o Waitangi.
