Job summary

Health New Zealand | Te Whatu Ora invites an experienced Registered Social Worker to join the Tiaho Mai team at Middlemore Hospital as a Transitions and Complex Care Coordinator. This new role supports admissions, discharge planning, and care coordination for patients with complex needs.

You will collaborate with Acute Community Services, NGOs, housing providers and whaanau to ensure safe, person-centred care and advance equitable health outcomes for Māori and all communities.

Qualifications

• Registration as a Social Worker with a current Annual Practising Certificate.
• At least five years' post-registration experience.
• Extensive experience supporting people with complex mental health and social needs.
• Strong knowledge of discharge planning, community pathways, and multidisciplinary practice.
• Excellent communication, negotiation, and relationship-building skills.
• A commitment to Te Tiriti o Waitangi and achieving equitable health outcomes for Māori and all communities.
• A full drivers licence.

Responsibilities

• Coordinate admissions, transitions, and discharge planning across inpatient and community services.
• Identify and address barriers that impact patient flow and recovery.
• Lead multidisciplinary and multi-agency care planning meetings for complex cases.
• Build strong partnerships with internal and external services to support equitable, person-centred outcomes.
• Monitor complex cases and contribute to service improvement initiatives that strengthen patient flow and system performance.
