Ōtorohanga Support House Whare Āwhina Trust is looking for a compassionate, organised and community-minded Case Management Kaiārahi Coordinator to join our team.
This is a hands-on role working directly alongside whānau to understand their needs, identify their goals and connect them with services, opportunities and community initiatives that can make a meaningful difference.
As our Case Management Kaiārahi Coordinator, you will be a key point of contact for whānau seeking support. You will build trusted relationships, coordinate support and work collaboratively with community organisations and service providers.
Your responsibilities will include receiving referral, engaging with whānau, supporting and developing plans, connecting whānau with other services, monitoring progress and providing ongoing support.
Throughout your work, you will engage with whānau in a professional, respectful and mana-enhancing way. The role is full time, Monday to Friday 9am to 4pm.
