Job summary

Access Community Health in the Hutt Region is seeking an Advanced Support Worker to join an experienced team delivering person-directed care across the region. You will champion client care and support staff through training and hands-on supervision.

You will liaise with tāngata, whānau, colleagues and partner agencies to coordinate care and ensure services are centred on what matters most to the person. The role involves hands-on work with tangata with complex needs and providing leadership to

Qualifications

• At least two years’ experience delivering disability, home-based or community support services.
• Full New Zealand driver’s license is required.
• Level 3 Health and Wellbeing Certificate or equivalent experience.
• Proficiency with Microsoft Office.
• Ability to work effectively in multidisciplinary teams.

Responsibilities

• Oversee how our support is delivered across the Hutt Region.
• Champion client care with our Support Worker teams and ensure staff competency.
• Liaise with tāngata, whānau, colleagues and partner agencies to coordinate care.
• Provide leadership, on-the-job guidance, and just-in-time support to less experienced staff.
• Work hands-on with tangata with complex support needs and contribute to service improvement.
