Job summary

Access Community Health invites applications for an Advanced Support Worker in New Zealand. You will work with care coordination and community nurse teams, performing non-complex client evaluations and delivering complex care under indirect supervision from the supervising clinician.

You will take on a leadership role, offering on-the-job and just-in-time training to less experienced workers and helping to develop the SW workforce within a multidisciplinary team environment including Palliative

Qualifications

• Experience or willingness to learn behaviour management is preferred.
• Proven ability to work in multi-disciplinary teams including nursing and care coordination.
• Must have a NZ driver's licence (full or restricted).
• Level 3 or 4 Health and Wellbeing qualification or equivalent.

Responsibilities

• Conduct non-complex client evaluations under supervision.
• Provide complex client care and carry out advanced tasks after training.
• Provide on-the-job and just-in-time training to less experienced staff.
• Support the care coordination and community nurse teams.
