Job summary

Access Community Health in Kapiti/Porirua region is seeking an Advanced Support Worker to join an experienced team delivering person-centred care across the Hutt Region. This newly created role offers close collaboration with tāngata and whānau to support care delivery and outcomes that matter to the person.

You will lead and mentor staff, coordinate with partner agencies, and support clients with complex needs.

Qualifications

• Experience delivering disability, home-based or community support services.
• Ability to work effectively within multidisciplinary teams (e.g., Palliative Care, Home-Based Nursing, Support Worker Coordination).
• Full New Zealand driver’s license.
• Level 3 Health & Wellbeing Certificate (or equivalent) and working towards Level 4.

Responsibilities

• Oversee how our support is delivered across the Hutt Region.
• Champion delivery of client care with trained and verified support teams.
• Provide leadership and on-the-job guidance to less experienced staff.
