Job summary

The Salvation Army is seeking a compassionate Support Worker for the Lifehouse - Southward Young Peoples Service to provide person-centred support to clients experiencing homelessness or vulnerability. You will develop individual support plans, assess needs, and coordinate access to housing, benefits and health services while maintaining accurate records.

Qualifications

• Demonstrated understanding of needs of homeless or vulnerable people.
• Excellent communication, written and verbal.
• Ability to work well in a team and coordinate with partners.

Responsibilities

• Develop, review and update individual support plans with clients.
• Complete client assessments including risk assessments within agreed timeframes.
• Help clients access benefits, housing options and health services.
• Coordinate and deliver in-house support activities to achieve positive outcomes.
• Maintain accurate records in the client management system and support team communication.
