Job summary

MSWA is seeking a compassionate Community Support Worker for a permanent part-time role (30 hours per fortnight) in Albany. You will provide person-centred support to participants with complex care needs and neurological conditions, helping them to maintain independence and quality of life.

Travel across Albany and surrounding suburbs is expected, with shifts between 6:00am–6:00pm or 7:30am–7:30pm, Monday to Friday.

Qualifications

  • Experience in person-centred support for individuals with complex care needs or neurological conditions.
  • Strong communication and interpersonal skills required.
  • Excellent record-keeping and reporting skills essential.

Responsibilities

  • Deliver person-centred support to participants in home and community settings.
  • Assist with personal care including showering, grooming, dressing, toileting, and continence care.
  • Support mobility, transfers and use of assistive equipment.
  • Administer or assist with medication per policies and training.
  • Facilitate appointments, community activities, and daily living skills.
  • Monitor wellbeing and report health/behaviour changes promptly.
  • Maintain documentation, progress notes and incident reports.

We are seeking a compassionate, reliable, and experienced Community Support Worker to join our team on a permanent part-time basis. This role involves providing person-centred support to individuals with complex care needs, neurological conditions, enabling them to achieve their goals and maintain independence, dignity, and quality of life. The successful candidate will have experience supporting participants with high care needs and neurological conditions and be committed to delivering exceptional care in accordance with individual support plans and organisational policies. This is a permanent part-time position offering 30 hours per fortnight. You will be required to travel throughout Albany and surrounding suburbs. You need to be available from Monday to Friday between 6:00am - 6:00pm or 7:30am -7:30pm.

Key Responsibilities
  • Deliver high-quality, person-centred support to participants in their home and community environments.
  • Assist with personal care, including showering, grooming, dressing, toileting, and continence care.
  • Support participants with mobility, transfers, and use of assistive equipment.
  • Provide support for individuals with neurological conditions such as acquired brain injury, multiple sclerosis, Parkinson’s disease, motor neurone disease, and other complex health needs.
  • Administer or assist with medication in accordance with organisational policies and training.
  • Support participants to attend appointments, access community activities, and develop daily living skills.
  • Monitor participant wellbeing and promptly report changes in health or behaviour.
  • Complete accurate documentation, progress notes, and incident reports.
  • Maintain professional relationships with participants, families, health professionals, and team members.
  • Adhere to NDIS Practice Standards, workplace health and safety requirements, and organisational policies.

Essential Requirements
  • Certificate III in Individual Support (Disability) or equivalent qualification.
  • Current First Aid and CPR Certificate.
  • Current NDIS Worker Screening Check.
  • Current Australian Driver’s Licence and reliable vehicle.
  • Ability to work independently and as part of a team.
  • Strong communication and interpersonal skills.
  • Excellent record-keeping and reporting skills.
