Job summary

Blue Swan Home Care in Kota Kinabalu is seeking a capable Client Care Coordinator to help coordinate patients, families and the day-to-day operations of our home-care service. This office-based role offers growth into a more senior client coordination position as the Blue Swan expands.

Responsibilities include coordinating bookings, communicating with patients and families, guiding new enquiries, coordinating clinicians, and supporting billing and records.

Qualifications

• Excellent organisation and communication with patients and families.
• Comfortable speaking with doctors and healthcare professionals.
• Ability to manage multiple patients and priorities simultaneously.
• Proactive, reliable and completes tasks to completion.
• Comfortable using computer systems and digital tools.

Responsibilities

• Coordinate patient bookings, home visits and appointments.
• Communicate care arrangements to patients and families.
• Guide new enquiries and explain next steps to families.
• Coordinate doctors, nurses, clinicians and caregivers.
• Follow up on care plans and appointments.
• Organise medications, equipment, supplies and logistics.
• Prepare and follow up on billing and payments.
• Maintain accurate patient records and admin docs.
• Share important changes with the clinical team.
• Resolve day-to-day scheduling issues.
• Support continuity of care from discharge to home care.
• Assist with coordination and administration of Blue Swan’s patients.
