Seamlessassist is seeking a Cardiology Patient Navigator (Coordinator) to support a U.S.-based cardiology team in a remote capacity. The role centers on patient communication, scheduling, care coordination, and clinical documentation, with a focus on accurate EMR/EHR entry and HIPAA compliance.

The ideal candidate has a clinical background (CNA, MA, RN or equivalent), 3+ years in healthcare or telehealth, bilingual English/Spanish, and comfort managing a high-volume caseload across multiple

Formación

• Clinical healthcare background required (CNA/MA/RN or equivalent).
• 3+ years in healthcare, telehealth, or medical office settings.
• Experience in care coordination or care management, preferably telephonic/remote.
• Fluent in English and Spanish (B2+).
• Experience using EMR/EHR systems and strong patient-centered communication.

Responsabilidades

• Manage patient scheduling, reminders, and follow-up workflows in EMR/EHR systems.
• Conduct telephonic or remote patient outreach to reinforce care instructions and coordinate next steps.
• Track testing schedules and remote patient monitoring within a high-volume caseload.
• Escalate clinical concerns to the appropriate provider within scope (no diagnosing).
