Pacific Health Group is seeking a dedicated housing stability professional to support community members with housing navigation and care coordination in field settings. The role emphasizes collaboration with housing authorities, landlords, shelters, and healthcare partners to secure and sustain stable housing for high-needs individuals.

You will conduct assessments, develop individualized care plans, and coordinate services across medical, behavioral health, and social supports.

Formación

• 3–5 years of case management, social services, or healthcare experience.
• Experience with Medi-Cal, CalAIM, and Community Supports programs (preferred).
• Strong communication, empathy, and cultural competence.
• Proficient with case management systems and real-time documentation.
• Ability to travel locally within the county to conduct in-person visits.

Responsabilidades

• Support housing placements and housing resource navigation.
• Coordinate Housing Tenancy & Sustaining Services for ongoing stability.
• Assist with Housing Deposits and required documentation.
• Collaborate with housing authorities, landlords, shelters, and service providers.
• Monitor housing stability and address barriers impacting health outcomes.
• Build partnerships and represent Pacific Health Group in the community.
